Appointments

Intake Evaluations                            $300.00

Follow-up                                          

15 minutes or less:   $75

30 minutes or less:   $150

45 minutes or less:   $225

60 minutes or less:   $250

 

Consultation                                      $300.00

(This includes a full evaluation and letter to referring provider)

Cancellation & No Show

There is no charge to reschedule an appointment 48 hours before the appointment itself.

 

Half fees are charged with less than 48 hours notice for the cancellation.

​

No show will be charged half fee for the scheduled appointment.

​

Additional Fees

Administrative Tasks                      $200 per hour

This includes writing a patient care summary, completion of lengthy forms, or other tasks that are requested by the patient outside of an appointment.

 

Legal Tasks                                       $450 per hour

Includes a multitude of items. See legal section for more details.

 

Form Completion                            $5 per page

Please note forms can take up to 7 business days to be completed. Bring appropriate paperwork to your appointment. Outstanding balances need to be addressed prior to the completion of forms.

Insurance Reimbursement

Medicare

I do not participate with Medicare and a claim for reimbursement can not be submitted to Medicare.

 

Medicare does not allow you to request reimbursement.

If you have Medicare, there is a form to sign indicating you are aware of their policy.

Commercial Insurance

The office will provide you a service receipt that can be submitted to your insurance for reimbursement.

​

We have no control over what your insurance company is willing to reimburse or cover.

​

Simply let us know if you need a receipt. 

Additional Information

I highly recommend you understand the role a Medicare plan plays when dealing with insurances.

 

It can be primary or secondary; and at times because of your Medicare insurance, bills can not be submitted to other agencies and therefore you are responsible for the entire bill.
